AT SCHOOL
The student appears to need directions repeated
The student appears to perseverate or get stuck doing the same activity over and over or saying the same thing or feeling the same emotion...
The student does not begin assigned tasks; does not initiate
The student does not complete assigned tasks; does not persist
The student does not do/refuses to do what he is told to do
The student appears to have difficulty attending (e.g., considerable off-task behavior)
The student engages in off-task and possibly distracting behaviors
The student rarely sits still
The student rarely generates new ideas
The student appears to have difficulty organizing materials, language, and ideas
The student becomes disorganized during transition times
The student appears to be forgetful (e.g., forgets homework, forgets information)
The student appears to act without thinking
The student's performance is inconsistent from day to day
The student hits or pushes other students or uses aggressive language
The student frequently appears angry
The student cries frequently; appears sad or depressed
The student appears to need continuous encouragement
The student can read (decode) words, but does not seem to understand what he reads
The student frequently gets stuck doing one thing, thinking one thought, or feeling one emotion
The student has difficulty with/resists writing tasks
The student appears to engage in attention seeking behavior
The student responds and works very slowly
